Fix migration UNIQUE constraint on BaseItemProviders
...
Deduplicate ProviderIds by ProviderId during MigrateLibraryDb migration
to prevent UNIQUE constraint violations when legacy data contains
duplicate provider entries for the same item.
Fixes #16134
